Death certificate states birth in Provo and death in Orem on the same day and at the same RFD #1 location, which is in Provo! The informant on his DC was his father, who also gave the same location as residence listed at same RFD# 1 in Provo, Utah. He would know where Baby Boy Shepherd was born—at home in Provo, Utah.
