According to his wife's obituary he was buried at the Brainard Cemetery, presumably the Bohemian National Cemetery. See the Butler County Press (David City, Nebr.), March 9, 1950, p. 1, col. 3. Son of Mathias and Frances (Mashek) Slavik. Married Antonia Urbanek on September 13, 1891 at Holy Trinity Catholic Church, Brainard, Nebr.
