http://www.rootsweb.ancestry.com/~neplatte/vitals/vitals1975.html The Columbus Telegram, October 10, 1975 KOZAK - Funeral rites will be held at 9:30 a.m. Monday at St. Anthony Catholic Church for James Edward Kozak, 30, Richland, who was killed in a car-train accident Thursday evening on the Platte-Colfax County road east of Columbus. The Rev. Laurin Buznski will officiate, with burial in st. Bonaventure Cemetery. Rosary service will be 8 p.m. Sunday at the McKown Funeral Home where friends may pay their respects after 1 p.m. Sunday. Mr. Kozak, an employe of the United Parcel Service, was born April 15, 1945 at Fullerton to Alphonse E. and Helen (Dubas) Kozak. He served in the Air Force from December, 1963 to December, 1967, attaining the rank of sergeant, and married Angeline A. Bialas at Scott AFB, Belleville, Ill. He was a member of St. Anthony Parish. Surviving are his wife; two children, Christine and James Jr.; his father and step-mother; brothers, Paul T. of Duncan, Dennis F. of Germany, Raymond L. and Thomas of Columbus; sisters, Mrs. Jim (Barbara) Smith of Silver Creek, Mrs. Kenneth (Dorothy) Liss of Duncan and Theresa Kozak of Columbus. His mother and three brothers preceded him in death.
