Transcription of Death Certificate: JOHN MIKULENCAK: Died in Hallettesville, Texas, at the home of Charles Michalik (where he had resided for 6 weeks) on May 19, 1964, at 12:15 a.m. Usual Address: Moulton, Lavaca County, Texas. Cause of death was listed as squamous cell carcinoma with metastasis diagnosed on June 27, 1963. At time of death, he was listed as married, a retired farmer, and 84 years of age. His birthdate was listed as January 15, 1880, birthplace Europe. His parents were listed as Paul Mikulencak and Anna Michalek. Informant: Agnes Mikulencak (Wife). Burial in the Moulton Catholic Cemetery on May 22, 1964, by Smith Funeral Home #3881. Attending MD: Indecipherable. Obituary: "The Lavaca County Tribune"—1964 JOHN MIKULENCAK A retired Moulton farmer, John Mikulencak, 84, passed away Tuesday, May 19, at the home of his daughter and son-in-law, Mr. and Mrs. Charles Michalec, in our city. He has been ill for some time. Born in the Old County, his parents came here when he was 4 years old. In 1916, he married Agnes Vanzura at Weimer, and they settled in the Hackberry section, farming until 1929. They moved then to the Moulton area, spending their remaining married life. The funeral was held Wednesday, May 20, from the Moulton Catholic Church, Father Wojciechowski officiating, with the interment in the Moulton Catholic Cemetery. Surviving are his wife; one son, Edward; and three daughters, Mrs. Vlasta Smetana of Schulenburg, Mrs. Frankie (Marcella) Galla of Ganado, and Mrs. Charles (Evelyn) Michalec of Hallettsville; also, four brothers, Joe of Hallettsville, Bill and Frank of Moulton, and Paul of Gonzales; also, one sister, Mrs. Mary Kahanek of El Campo.
