An allopath, he was educated at Emory University School of Medicine, Atlanta, Georgia, graduating in 1882. He resided in Center for about 25 years, where he was a practicing physician, beginning in 1882. He married Lillian Blakey on May 20, 1883, in Nacogdoches County, Texas, and she died in 1890. His daughter Pearl Leak Payne died on January 6, 1909, and in 1910 their 3-yr-old granddaughter, Frances Payne, was living with Erastus E., Kate E., and Onita, 17. Dr. Leak retired about 10 years before his death. After his wife died in 1916, he moved to San Augustine to live with his daughter, Winfrey L. (Mrs. Rayford N.) Stripling.
