John Warren Forman was born November 25, 1918, in Omaha, Douglas County, Nebraska, the son of George Oran and Olga Mae (Fight) Forman. After High School, John attended and graduated from University of Omaha (Nebraska) where he studied Mathmatics. John served with the U. S Navy during World War II from April 1942 to March 1946. John married Betty Lu Huston on December 26, 1954, in Bellevue, Sarpy County, Nebraska. Betty was born December 9, 1923, in Tecumseh, Johnson County, Nebraska, the daughter of Harry Edwin and Lena Letitia Black. John and Betty had the following known children: 1. David Huston Forman (1960-2019) 2. Elizabeth Jane (Forman) Bolens 3. Matthew John Forman John died May 22, 1998, in Endicott, Broome County, New York. Betty died July 22, 2013, in New York.
