He is predeceased by his spouse Joann G Ondrak, and daughter Bambi Ondrak. He is survived by sons Keven (Jodi) Ondrak of South Carolina, Kyle Ondrak of West Valley City Utah, daughter Tami Sablan of West Valley City Utah, grandchildren, and many nieces and nephews. No formal services will be held at this time. In lieu of flowers please donate to the Alzheimer's Foundation.
