Theodore Perry Malquist Loving husband and father, passed away March 30, 2005 in Provo, Utah. Born May 8, 1916 in Salt Lake City to Edward Theodore and Martha Perry Malquist. Ted lived most of his life in Lakewood, California before retiring to Provo. Ted is survived by his 2nd wife, 3 sons and their families. Preceded in death by his first wife, Alice Clark Malquist. Funeral services will be held at 2 p.m. Friday, April 1, 2005 in the Provo Grandview 15th Ward, (1122 Grand Ave, 1 block south of the family residence). Viewing from 12 noon to 1:45 p.m. prior to the services. Interment in the Alpine City Cemetery. Published in the Daily Herald , Provo, Utah on 3/31/2005.
