According to her death certificate she was born in Czechoslovakia, and was widowed at the time of her death. (Her parents' names were not known or not recorded.) The informant on her death certificate was Frank Wentrcek. According to the death certificate of her son, Louis Richard Wentrcek , Annie's maiden name was also Wentrcek. (There was an explanatory note in the margin of the death certificate that her maiden name was accurate.)
