Bio

Dr. Leo Ladislav Polak, DDS, son of Ivan and Frances (Sedlak) Polak, was born June 28, 1932, at Brainard, Nebraska. Leo died Saturday, March 2, 2013, at Avera Creighton Hospital in Creighton, Nebraska. Leo attended Holy Trinity school and graduated from Brainard (NE.) High School. He then attended and graduated from Creighton University Undergraduate School, and began teaching high school math and science at Howells, Nebraska. In 1955, after three years of teaching at Howells, Leo served in the United States Army, until receiving his Honorable Discharge in 1957. Upon returning from the service, Leo enrolled in the dental school at Creighton University, and graduated with honors in 1964. Following graduation, Leo moved to Creighton, Nebraska, where he practiced for 35 years as a Dentist; first with Dr. Roman C. Roh, and for the next 33 years in his own Dental Clinic, until retireing due to health issues. On May 1, 1965, Leo was married to Elizabeth Walz at St. Bernards Catholic Church in Omaha, Nebraska. The couple was blessed with six children. Leo was a member of St. Ludger Catholic Church in Creighton, where he was involved with the Knights of Columbus, was a CCD Teacher, and served on both the Parish Council and the Finance Committee. He also served on the Creighton School Board, on the Library Board, and the Creighton Chamber of Commerce. Leo is survived by: wife, Elizabeth Polak of Creighton, Nebraska; children, Lori (husband Van) Wewel of Gretna, Nebraska (their children, Katherine and Lizabeth), Tom (wife Sally) Polak of Sioux Falls, South Dakota (their children Holly and Morgan), Mike (wife Loraine) Polak of Pierre, South Dakota (their children, Ashley, Sarah, and Isaac), Rob (wife Laura) Polak of Gretna, Nebraska (their daughter, Alexis), Jim (wife Shonna) Polak of Chesterfield, Michigan (their children, Kyle and Camdin), and Matt; brother, Dick (Rita) Polak of David City, Nebraska; and sisters, Georgia (husband Phil) Siraguza of Omaha, Nebraska, Delores Cimino of Omaha, Rosemarie (Denny) Rosenberger of Denver, Colorado, and Sr. Joan Polak of Omaha. Leo was preceded in death by: parents, Ivan and Francis Polak; step-father, Fred Kriz; brother-in-law, Jim Cimino; and nephew, Jimmy Lee Cimino. Burial was in the St. Ludger Catholic Cemetery, Creighton, NE.
