According to his death certificate he died at 4013 Trowbridge St., Cleveland, on November 30, 1914 and was buried in St. Mary's Cemetery on December 3. Birth date from death certificate. ''''''''''''' Brainard Clipper (Brainard, Nebr.), December 3, 1914, p. 5, col. 3 Bruno Department Joseph Ptacek, who has resided near Bruno for over twenty years, and father of Anton, James and Joseph, jr., died Monday night at the home of his daughter, Mrs. Ptak in Cleveland, Ohio, at the advanced age of about 90 years. A telegram was received here Tuesday morning announcing his death and his son Joseph departed the same day for Cleveland to be present at the funeral.
